Bristol Royal Infirmary - CHD - Following a successful
year-long pilot, patients who are taken to the Bristol Royal Infirmary with a
heart attack will now be treated by angioplasty rather than thrombolysis.
Immediate angioplasty will now be available 24 hours a day, 7 days a week. It
is estimated that if all heart attack patients in the UK received immediate
angioplasty, there would be 500 fewer deaths per year, 1,000 fewer repeat heart
attacks and 250 fewer strokes. Dr Andreas Baumback, Lead Consultant
Cardiologist, said the new service would also cut the time patients spend in
hospital, as there is less damage to the heart.

Bournemouth and Poole PCT - Heart Failure
- The Better Together service is a joint initiative between the PCT, Marie
Curie Cancer Care and the British Heart Foundation (BHF) which was launched
last September. It aims to provide the best possible care for heart failure
patients and allows them to be cared for at home in the final stages of their
illness. Elaine Rouse, Poole BHF Heart Failure Nurse, said that one of the
benefits of the scheme was that she now felt more aware and confident in
dealing with end-of-life issues with patients. The main expense is the cost of
a Marie Curie nurse on night shift, and this is split between the local PCT and
the BHF. It is hoped that the year-long pilot will pave the way for more joint
working on diseases such as COPD.

Cumbria PCT - Diabetes Care for the Elderly - A project based
at Cumberland Infirmary which aims to improve the lives of elderly people with
Diabetes has won the Arun Baksi Award for Team Innovation in Diabetes Care at
the annual Diabetes Conference on the Isle of Wight. The project was started
after an audit highlighted the prevalence of diabetes in nursing and
residential care homes, and the need for extra staff training.

Brent tPCT / Harrow PCT - Joint Programme Arrangements - It has
been proposed that the two PCTs, Practice Based Commissioners and North West
London Hospital Trust work together to develop their individual healthcare
strategies through a single programme arrangement. A public consultation
process is underway.

Bath - New Mini Hospital - Proposals have been announced for a
new mini-hospital serving Bath, based at Peasedown St John. The new facility
would be a co-operative, co-owned by all its staff,and it would treat both
private and NHS patients. The new unit would specialise in day surgery,
providing a range of treatments, including ophthalmology and orthopaedics,
although a number of patients would be able to stay overnight. It would have
four operating theatres, an endoscopy suite, 26 in-patient beds and radiology
and physiotherapy departments, with a capacity to treat around 600 patients per
week.

Bedfordfordshire
Heartlands PCT - Sexual Health Survey - The PCT's website is hosting an
anonymous internet survey on sexual health from 25th Sept to 8th Oct. Services
they are interested in hearing about include contraception, sexually
transmitted infection services, and information/advice.

Bexley Care Trust - Diabetes - Concern
has been expressed by local Diabetes groups regarding the future provision of
services at Queen Mary's Hospital, which provides 80% of diabetes care for
Bexley Care Trust. The hospital has proposed a 50% reduction in services from
its Centre of Excellence Diabetes Unit - this includes Patient Education, the
loss of the inpatient Specialist Nurse, and the Paediatric Specialist Nurse
being cut to half the full-time equivalent. Bexley Care Trust also has
financial difficulties which, it is feared, will compound the
situation.

Barnet PCT / Barnet,
Enfield & Haringey Mental Health Trust - Barnet, Enfield & Haringey is
proposing to reorganise its mental health services so that patients with mild
to moderate mental health problems will be seen by their GP. The GPs will, in
turn, be supported by teams of dedicated specialists, including psychiatrists,
psychologists and social workers. It is thought that approximately 3,000 people
would fall into this category.

Brent PCT - Diabetes Project - Brent has a higher than average
incidence of diabetes, so most healthcare professionals deal with the disease
every day. The Brent Emergency Care and Diagnosis Centre (BECaD) Single Point
of Access Diabetes Project went live in March and the PCT is now hosting a
launch event for GPs and AHPs. The decision of some of the newly formed
practice based commissioning (PBC) groups to make diabetes a priority reflects
and acknowledges the importance of this potentially serious
condition.

Blackburn with Darwen
PCT - Integrated Medicines Management Programme - Stakeholders from the PCT,
Social Services, voluntary services, education, GP practices and community
pharmacy have attended the first meeting of the PCT's new Integrated Medicines
Management Programme for People with Long-Term Conditions. Blackburn with
Darwen is one of 25 organisations across the country participating in the
National Prescribing Centre's IMMP project. It is designed to help NHS
organisations implement an integrated approach to medicines management, thus
supporting people with long-term conditions across whole health communities.

Ashford PCT - Integrated
Clinical Assessment & Treatment - The Ashford Integrated Clinical
Assessment & Treatment Services (ICATS) is at the heart of the PCT's
strategy to develop more services within primary care, and is currently being
piloted at the West View Intermediate Care Centre in Tenterden. GPs are now
able to refer patients for triage, assessment and treatment and then, if
necessary, into secondary care. Clinical care pathways have recently been
revised within orthopaedics, chronic pain and rheumatology, resulting in
combined pathways for musculoskeletal care. ICATS will be accepting referrals
for hips and knees in the first instance, a back care pathway will be
implemented during 2006 and chronic pain services will be developed alongside
the pilot.

Cambridge City /
South Cambs PCTs - National Appointment - Dr Mike Knapton, a GP at the Nuffield
Road Medical Centre, has become the first Director of Prevention and Care at a
the British Heart Foundation. He will no longer be the Acting Medical Director
for the PCTs, but will continue as a GP for one day a week.

Bassetlaw PCT - Organisational Changes -
PCT board members have supported a proposal to link Bassetlaw with Doncaster
rather than other PCTs in Nottinghamshire, because of the PCT's strong clinical
and geographical links with South Yorkshire.

Buckinghamshire Mental Health NHS Trust - Proposed
Changes to Services - Proposals for changes in the delivery of mental health
services in Buckinghamshire aim to improve hospital services for the most
vulnerable, while increasing the proportion of care which is given outside
hospital. The proposals will be rolled out over the next three years, and
include: seven-day acute hospital services in Aylesbury and High Wycombe, an
expanded home treatment capacity and in-patient units for people over and under
65, more community support for non-hospital accommodation for people with
long-term needs - delivering day care for older people with dementia in the
Buckingham area with the help of voluntary organisations.

Derby Medical
Services (DMS) has negotiated a new contract, worth more than £6m, with
southern Derbyshire's five PCTs to provide out-of-hours GP care for the area.
In July, it claimed that patients' lives would be put at risk if it could not
nearly double its £3.5m budget. The new contract is back-dated to
April.
DMS has also won a 5-year £5m contract to provide a
specialist medical service for detainees held in custody by the Derbyshire
Police Force. An amendment to legislation means that nurses are now allowed to
work in custody suites, and DMS will employ 17 nurses at four custody suites,
including 12 at St Mary's Wharf in Derby. It is also hiring three more GPs. DMS
is hoping to further improve services by involving agencies like Addaction. The
changes have more than halved the cost of the service.

Co Durham &
Darlington Priority Services / Tees & NE Yorkshire NHS Trusts - Merger
Consultation - A consultation has been launched by the two NHS Trusts on their
proposal to merge and create a single organisation to provide all mental
health, learning disability and addictive behaviour services in the County
Durham, Tees Valley and NE Yorkshire area.

Carlisle
/ West Cumbria / Eden Valley / Morecambe Bay PCTs - Reorganisation - Cumbria
& Lancs SHA has begun a consultation exercise on local reorganisation which
will be presented to the DoH in October for implementation 2006. Proposals
include the merging of 4 Cumbrian PCTs and the creation of a regional NW Health
Authority.

Birmingham and the Black Country SHA - Chief Executive - David
Nicholson, Chief Executive of Birmingham and the Black Country SHA, has also
been appointed Chief Executive (designate) at Shropshire and Staffordshire SHA
(from 1 August) and West Midlands South SHA (from 1 September).

The Quality &
Outcomes Framework that was such a key feature of the new GMS contract has been
a huge success story for British General Practice with an achievement rate of
over 90% of all targets now certain.The cynics will say that the targets were
too easy, the GP negotiators too smart, that points have rewarded existing good
practice described in a different way, and that the pressure on PCT budgets
outstripped available funding by £200 million pounds by the end of
2004/5. The fact remains that the Government set targets and Practices
responded. Even the cynics will have to concede that the main aim of
demonstrating change in clinical practice by the use of financial levers has
worked. As one commentator rather crudely remarked the pigs are in the
pen, now lets change the shape of that pen.

Bedfordshire
Heartlands PCT - Wound Care - District nurses at Bedfordshire Heartlands PCT
have been using maggots to treat wounds for a number of years, and are now
encouraging local patients to volunteer for a nationwide study which will
compare their effectiveness with conventional dressings.

The Government has set a Public Service Agreement
target to address geographical inequalities in life expectancy, cancer, heart
disease, stroke and related diseases. The targets aim to see faster progress
compared to the average in the fifth of areas with the worst health and
deprivation indicators. Achievement of the targets will be assessed on the
outcomes for this Group in 2010. The Local Authorities and Primary Care Trusts
which are in these areas, the so-called Spearhead Group is made up of 70 Local
authorities and 88 Primary Care Trusts, based upon the Local Authority areas
that are in the bottom fifth nationally for three or more of the following
indicators: Male life expectancy at birth, Female life expectancy at birth ,
Cancer mortality rate in under 75s, Cardio Vascular Disease mortality rate in
under 75s, Index of Multiple Deprivation 2004 (Local Authority Summary),
average score

PCT Mergers - During the last year a number PCTs have
combined with their neighbours to share a Chief Executive, a management board
or even mergers of functions at lower management levels. A number of PCTs now
share Chief Executives. Some mergers have been initiated by the SHA (for
example Hampshire and Isle of Wight SHA) or because one of the PCTs was
struggling (Chelmsford PCT - a zero star rated PCT in 2003/4 now sharing a
Chief Executive with Witham, Braintree and Halstead Care Trust) or because of a
long history of collaborative working (North and South Peterborough PCTs.)

Chorley and
South Ribble PCT - Substance Misuse - A new Substance Misuse Support Centre is
to open this month in Golden Hill Lane. A team of specialist nurses, doctors,
substance misuse and social workers will be stationed at the centre and the
South Ribble Alcohol Service will be relocated there.

Barnet PCT -
Disabled Access - A local group, Disability Action borough of Barnet (DAbB),
has visited 12 clinics in the Barnet area and claim that the majority do not
meet the new Disability Discrimination Act standards which come into force in
October.

Burnley, Pendle and Rossendale PCT - CHD Information
Event - The PCT launched its new Heart Disease Resource Pack for health
professionals at a recent CHD Information Event. Over 350 professionals,
including 80 GPs, attended the event which included presentations on subjects
such as statin prescribing and diet.

Central Cornwall PCT - Eldercare Project in Cornwall
(EPIC) - The Eldercare Project in Cornwall (EPIC) is a new county-wide
preventative nursing service which will provide assessment and intensive case
management to elderly patients who are at risk of unplanned
hospitalisation.

Bassetlaw PCT - Specialist Support Night Nursing Team - This
team has been running for almost a year and has prevented many unnecessary
admissions to hospital for terminally ill patients. Nurses are now undertaking
training for extended roles following government legislation on out-of-hours
services.

Camden, Islington and Haringey PCTs - Lifeforce Team - Three
PCTs have launched the Lifeforce Team to provide a palliative care/bereavement
service for children. The specialist paediatric team comprises a specialist
nurse, youth worker, respite nursery nurse, play specialist, doctor and
clinical psychologist.

Durham Dales PCT - Online GP Appointments - Durham Dales PCT is
in the early stages of piloting a new system whereby patients can book
appointments with their GP via the internet. The system is operating in
Bishopgate Medical Centre, and two further practices are investigating its
introduction.
